Job summary

Qualifications

• Active RN license in practice state.
• Minimum 2 years healthcare experience with care management or LTSS.
• Experience with persons with disabilities or chronic conditions preferred.
• Proactive, detail-oriented, and able to work independently.

Responsibilities

• Perform comprehensive member assessments within required timelines.
• Coordinate waiver enrollment/disenrollment and develop care plans.
• Monitor care plan effectiveness and adjust as needed.
• Promote integrated services across LTSS and behavioral health.
• Assess medical necessity and authorize waiver services.
• Facilitate ICT meetings for service decisions and collaborate with teams.
• Educate and motivate members using motivational interviewing.
• Identify barriers to care and support psycho/social, financial, and medical needs.
• Travel 25-40% locally as required by state/contract.
