Job summary

Molina Healthcare, Inc. is hiring a Registered Nurse for care management and coordination of LTSS across multiple counties in OH. The role collaborates with a multidisciplinary team to deliver integrated member care and improve outcomes.

You will conduct home visits, assess needs, and guide waiver services while navigating regulatory timelines. The ideal candidate has at least 2 years in health care with care management experience, an active RN license, and a valid driver’s license.

Qualifications

• RN license active and unrestricted in state of practice.
• Minimum 2 years in health care with care management or LTSS exposure.
• Experience with disability, chronic conditions or substance use disorders preferred.
• Valid driver's license and reliable transportation required.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, and clinicians.
• Monitor care plans and adjust interventions to achieve goals.
• Coordinate LTSS and behavioral health services with the care team.
• Assess medical necessity and authorize appropriate waiver services.
• Participate in ICT meetings and communicate decisions clearly.
• Travel locally 25-40% as required by state/contractual needs.
