Qualifications

• Registered Nurse (RN) with active license in state of practice.
• Minimum 2 years in health care with care management or LTSS exposure.
• Experience working with persons with disabilities or chronic conditions is preferred.
• Strong verbal and written communication skills required.
• Willingness to travel 25–40% locally as needed.

Responsibilities

• Perform comprehensive member assessments within required timelines, including home visits.
• Coordinate waiver enrollment/disenrollment and develop care plans with teams.
• Monitor care plans and adjust interventions to meet member goals.
• Promote integrated care across behavioral health and LTSS.
• Collaborate with ICT meetings to approve or deny services.
