Job summary

Molina Healthcare is seeking a Registered Nurse to support care management and care coordination for LTSS members in Ohio. The role involves conducting comprehensive assessments, coordinating waiver services, and developing care plans with input from members, caregivers, and clinicians.

Expect to travel locally (25-40%), collaborate with multidisciplinary teams, and ensure care is patient-centered, compliant, and cost-effective. Requires active RN license and prior managed care experience.

Qualifications

• RN license active and unrestricted in state of practice.
• At least 2 years health care experience including care management/managed care.
• Experience with persons with disabilities, chronic conditions, LTSS, or equivalent.
• Valid driver's license and reliable transportation for travel requirements.

Responsibilities

• Complete comprehensive member assessments within timelines, including in-person visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor care plans and document interventions and goal achievement.
