Job summary

Molina Healthcare is seeking a registered nurse to support care coordination for LTSS members across multiple counties in Ohio. The role involves conducting comprehensive assessments, coordinating waiver services, and developing individualized care plans with input from members, caregivers, physicians, and the care team.

The position requires 2+ years in health care with care management experience, travel up to 40%, and strong communication skills.

Qualifications

• RN license active and unrestricted in state of practice.
• At least 2 years in health care with care management experience.
• Ability to travel 25-40% as required by state/contractual needs.
• Strong verbal and written communication and interpersonal skills.
• Proactive, detail-oriented, and able to work with diverse populations.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care networks.
• Monitor care plans, document interventions, and adjust goals as needed.
• Promote integration of LTSS, behavioral health, and community resources.
• Assess medical necessity and authorize waiver services.
• Coordinate ICT meetings for service approvals and denials.
• Provide education and motivation during member contacts.
• Identify barriers to care and address psycho/social/financial obstacles.
• Manage potential critical incidents and develop prevention plans.
