Job summary

Molina Healthcare is seeking an RN Care Manager to support care coordination for LTSS across multiple Ohio counties. The role collaborates with a multidisciplinary team to ensure member progress toward outcomes and integrates services to optimize quality and cost-effectiveness.

The position requires an active RN license, 2+ years in care management, and travel up to 40% within the state. Competitive benefits are available for this full-time role.

Qualifications

Active RN license in state of practice.
At least 2 years health care experience with care management.
Experience with LTSS or disability populations preferred.
Valid driver's license and reliable transportation.
Able to travel 25-40% locally.

Responsibilities

Complete comprehensive member assessments within timelines, including home visits.
Coordinate waiver enrollment/disenrollment processes.
Develop and implement care plans with members, caregivers, physicians and other health professionals.
Monitor care plan effectiveness and adjust interventions as needed.
Educate and motivate change using motivational interviewing.
Facilitate interdisciplinary care team (ICT) meetings for approvals/denials.
