Mercy is seeking a Care Manager to support discharge planning from admission to discharge in collaboration with the health care team. You will coordinate transitions and assist patients with complex psychosocial needs, ensuring care aligns with Mercy standards.

The role requires an acute care nursing background, an Illinois RN license, and a BSN or higher. Strong communication and computer skills are essential in a fast‑paced hospital environment.

Qualifications

• Graduate of an accredited School of Nursing.
• Current License in the state of employment (Illinois).
• Bachelor's Degree of Science in Nursing, minimum required.
• 2–3 years in acute care hospital setting preferred.
• Care Management or Utilization Management experience preferred.
• BLS (CPR) at hire date or within 90 days; Case Management certification preferred.

Responsibilities

• Facilitates discharge planning from admission to discharge with the healthcare team.
• Intervenes with patients with complex or high‑risk psychosocial needs as assigned.
