Job summary

Somatus, a leading care provider, seeks an RN Care Manager, VBC in the Baton Rouge area to support high-need CKD/ESKD patients. You will partner with nephrologists and PCPs, visit members at home, dialysis clinics, and offices, and build individualized care plans to improve outcomes.

You will educate provider practices, coordinate interdisciplinary meetings, and travel locally to support coordinated care, with a strong emphasis on patient-centered outcomes.

Qualifications

• RN license and ability to obtain multi-state licensure as needed.
• 2+ years RN experience in a multi-disciplinary team.
• Valid BLS certification from an approved facility.
• Renal, CKD or dialysis care experience preferred.
• Willingness to travel across the assigned region.

Responsibilities

• Partner with physicians and practice staff to improve patient outcomes.
• Plan, coordinate, and facilitate regular interdisciplinary care team meetings.
• Establish relationships with patients and provider offices through in-person and telephonic engagement.
• Develop partnerships with provider practice teams to support clinical and operational goals.
• Collaborate to optimize workflows aligned with care team processes.
• Educate provider practices on the Somatus program and integrated workflows.
• Provide complete care continuum via in-person, telehealth, and phone interactions.
• Travel to member homes, facilities, and clinics for visits and care planning.
• Identify barriers to care through nursing assessment and address needs.
• Develop individualized care plans with nephrology and interdisciplinary team.
• Manage transitions of care and minimize readmissions.
• Ensure patient knowledge of discharge requirements and renal condition.
• Oversee tasks by non-licensed and licensed team members.
• Perform other duties as assigned.
