Job summary

Somatus is seeking an RN Care Manager in the Phoenix area to support high-need CKD/ESKD patients. You will work with nephrologists, PCPs, and care teams to plan, educate, and coordinate care across home, clinic, and facility settings.

Responsibilities include care planning, patient education, and collaboration with provider offices to improve outcomes. Travel will be 75% of the time within the assigned region.

Qualifications

• RN license and ability to obtain licensure in other states as needed.
• 2+ years of RN experience in a multi-disciplinary setting and with physicians.
• Valid BLS certification from AHA or Red Cross.

Responsibilities

• Partner with physicians and practices to improve patient outcomes.
• Coordinate care planning and interdisciplinary care team meetings.
• Travel to member homes, clinics, and facilities to deliver care coordination.
