Mercy Health is seeking an RN Care Manager for Fairfield Hospital to coordinate patient care across the continuum and promote safe, timely transitions. The role emphasizes collaboration with the healthcare team to assess needs, plan interventions, and evaluate outcomes.

Requirements include a BSN, BLS, and active RN license in the state. At least 1 year in a clinical setting is required; acute care and post-acute care coordination experience are preferred.

• Follow standardized Advance Care Planning, length of stay management, and readmission prevention processes.
• Support proactive care and remove barriers to timely testing and treatment.
