Qualifications

• Bachelor of Science in Nursing required or preferred (BSN)
• 1 year of clinical experience required
• 3 years of acute care experience preferred
• Ambulatory or post-acute care coordination experience preferred

Responsibilities

• Identify and prioritize patients needing care management using holistic factors.
• Document plans of care clearly in the patient record and ensure accurate handovers.
• Coordinate transitions across care settings and collaborate with the healthcare team.
• Support and promote timely testing, treatment, and discharge planning.
• Ensure resources are used appropriately and barriers to progression are addressed.
