Job summary

• Bachelor of Science in Nursing is preferred; active RN license is required.
• ACM/ CCM/ ANCC certifications are preferred for case management.
• Experience in clinical and acute care settings with care coordination is desired.

• Identifies and prioritizes patients in need of care management using holistic factors.
• Plans with patient, caregivers and healthcare team to optimize outcomes.
• Completes documentation reflecting plan of care and transitions.
• Supports discharge planning and readmission prevention.
