Mercy is seeking a Care Manager to join the interdisciplinary team. You will assess, plan, advocate, and coordinate care from admission to discharge to ensure safe transitions post-hospitalization.

Requirements include RN from an accredited school, current state license, and BLS. 2–3 years acute care experience preferred; CM/UM experience preferred. Mercy offers comprehensive benefits and a supportive work environment.

Formación

• Graduate of an accredited School of Nursing, required.
• 2-3 years acute care hospital setting, preferred.
• Care Management or Utilization Management experience, preferred.
• Current License in the state of employment, required.
• BLS (CPR) at hire date, required, or within 90 days of hire
• Certification in Case Management, Preferred.

Responsabilidades
