Job summary

Wellstar Health System in Georgia is seeking a Care Coordinator RN (CC RN) to assess transitional care needs and coordinate care across the continuum. You will engage patients and families, collaborate with physicians and the interdisciplinary team to ensure timely discharge and efficient use of resources.

The role requires an RN with at least 1 year of acute care experience, active state license, and BLS certification.

Qualifications

• Associates Nursing or Diploma in Nursing; Bachelor’s in Nursing preferred.
• Minimum 1 year nursing experience in the acute care setting.
• RN license (state) required; BLS certification required.

Responsibilities

• Assess transitional care needs and plan discharge.
• Coordinate care across the continuum with the care team.
• Engage patients and families to ensure goals and discharge plans are met.
• Collaborate with physicians, UM and utilization review for efficient care.
