Wellstar Health System in Georgia is seeking a Care Coordinator RN (CC RN) to assess transitional care needs and coordinate care across the continuum. You will engage with patients and families to ensure discharge planning aligns with goals, optimize resources, and facilitate timely transitions.

The role involves collaborating with the care team, physicians, and discharge planners, conducting psychosocial assessments, and documenting progress in the electronic medical record.

Responsabilidades

• Assess transitional care needs and psychosocial risk factors.
• Coordinate care across the continuum and with families.
• Lead discharge planning and ensure timely progression to discharge.
• Participate in interdisciplinary rounds and care conferences.
• Document in EMR and track avoidable days and trends.
• Serve as preceptor or mentor for student interns (if appropriate).
