Job summary

Responsibilities

• Assess transitional care needs and psychosocial factors for discharge.
• Collaborate with physicians and care team to support progression and discharge plans.
• Facilitate patient/family education and care conferences.
• Identify and address barriers to discharge; participate in interdisciplinary rounds.
• Document assessments and care plans in the medical record.
