Job summary

Wellstar Health System in Georgia is seeking a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate care across the continuum, and engage with patients and families to ensure discharge readiness.

You will work with physicians and the care team to remove barriers, document care in the EMR, and participate in rounds. Requires nursing licensure, BLS, and at least 1 year of acute care experience.

Qualifications

• RN licensure in Georgia or multi-state compact (NLC).
• BLS certification required.
• Minimum 1 year nursing experience in acute care.
• Excellent written and verbal communication.
• Knowledge of discharge planning and care coordination.

Responsibilities

• Assess transitional care needs and plan discharge.
• Coordinate care across the continuum with the care team.
• Engage patient and family to ensure needs are met.
• Collaborate with physicians and UM/IR teams on care progression.
• Document care and discharge plans in EMR and track progress.
• Participate in interdisciplinary rounds and ongoing professional development.
