Job summary

Qualifications

• Associates Degree in Nursing or Diploma; BSN preferred.
• RN license (Single State or Compact) and BLS or equivalent.
• Minimum 1 year nursing experience in the acute care setting.
• Excellent written and verbal communication; strong organization and problem-solving.

Responsibilities

• Assess transitional care needs and coordinate across the continuum.
• Engage patient and family to assure care needs are met and discharge planning.
• Collaborate with the care team to optimize patient progression and discharge.
• Document care plans and track discharge barriers.
