Job summary

Wellstar Health System in Georgia is hiring a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate across the continuum, and engage patients and families to ensure discharge readiness. The role partners with physicians, the care team, and a social work/UM component to optimize resource use and patient outcomes.

Minimum qualifications include an Associate Degree in Nursing or Diploma, RN license (or Compact), BLS, and at least 1 year of acute care nursing.

• Knowledge of local, state and federal regulations and community resources.

Responsibilities

• Assess patients' transitional care needs and psychosocial risk factors.
• Coordinate discharge planning and post-discharge referrals with the care team.
• Collaborate with physicians, UM/nurse teams, social workers and family to ensure timely discharge and appropriate care levels.
• Document assessments and care plans accurately in the EMR and track avoidable days.
