Job summary

Wellstar Health System is seeking a Care Coordinator RN to assess transitional care needs and coordinate care across the continuum. The role focuses on discharge planning, patient/family engagement, and collaboration with the care team to ensure timely and efficient care transitions.

The ideal candidate has at least 1 year of acute care nursing experience, strong communication skills, and the ability to navigate insurance and payer requirements.

Qualifications

• Minimum 1 year nursing experience in the acute care setting.
• Strong communication, interview and organizational skills.
• Knowledge of hospital discharge planning and care coordination.
• Ability to work with physicians, care teams and families.

Responsibilities

• Assess patients' transitional care needs and plan discharge.
• Coordinate care across the continuum and engage patient/family.
• Collaborate with PAS, financial counselor and UM nurse on insurance needs.
• Meet with care teams to review progress and discharge planning.
• Document accurately in medical records and track discharge milestones.
• Participate in interdisciplinary rounds and post-acute referrals.
