Job summary

Wellstar Health System in Georgia is seeking a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate across the continuum, and engage with patients and families to meet discharge goals.

You will collaborate with physicians, case managers, and the care team to optimize care progression, manage length of stay, and facilitate timely transitions, while ensuring compliance with accrediting standards.

Qualifications

• Associates degree in nursing or nursing diploma required; BSN preferred.
• RN license for single state or compact (multi-state) license.
• Current BLS/ARC-BLS certification required upon hire.
• Minimum 1 year nursing experience in acute care setting.

Responsibilities

• Assess patients' conditions and psychosocial needs to plan discharge and transitions.
• Coordinate with PAS, financial counselor, UM nurse to ensure insurance requirements are met.
• Collaborate with physicians and care team to develop and implement discharge plans.
• Participate in interdisciplinary rounds and post-acute referrals to optimize care progression.
