Qualifications

• RN license required or active nursing license in scope of practice.
• Experience in acute care and care coordination preferred.
• Strong communication and collaboration with care team and families.

Responsibilities

• Assess transitional care needs and psychosocial factors.
• Coordinate care across the continuum to meet patient goals.
• Engage patient and family to ensure discharge planning success.
• Collaborate with utilization review and physicians for appropriate care.
• Facilitate interdisciplinary rounds and plan of care progression.
• Identify barriers to discharge and escalate as needed.
