Responsabilités

• Assess chronic disease management needs and psychosocial risk factors at admission.
• Collaborate with care team to establish discharge plans and timelines.
• Coordinate post-acute referrals and ensure patient/family education and engagement.
• Document care progression and barriers to discharge; participate in interdisciplinary rounds.
