Job summary

Hartford HealthCare Organization is seeking a licensed RN care coordinator for per diem weekend coverage and PTO relief at MidState Medical Center. You will assess patients across the care continuum, develop discharge plans, and work with an interdisciplinary team to remove barriers to post-acute care access.

The role emphasizes clinical leadership, efficient use of hospital days, and ensuring adherence to the patient’s care plan. Excellent communication and problem-solving are required.

Qualifications

• Graduate from an accredited school of nursing.
• Previous experience in acute care healthcare setting.
• Valid Registered Nurse License with the State of Connecticut.
• Case Management Certification preferred.
• Preferred bilingual English/Spanish language skills.

Responsibilities

• Develops discharge planning within an interdisciplinary healthcare team.
• Identifies and removes barriers to post-acute care access.
• Collaborates with providers to optimize use of acute care hospital days.
• Provides clinical leadership to healthcare teams and ensures daily goals are met.
• Facilitates seamless transitions of care for patients and families.
