Job summary

Hartford HealthCare, MidState Medical Center in Connecticut, seeks an RN care coordinator for per diem weekend coverage. You will assess patients, develop and implement discharge plans, and work within an interdisciplinary team to address barriers to post-acute care.

Responsibilities include collaborating with providers to optimize hospital days, leading care teams, and ensuring goals are achieved per patient plans. CT RN licensure is required, bilingual in English/Spanish preferred.

Qualifications

• Graduate from an accredited school of nursing.
• Current Registered Nurse license in CT.
• Case Management Certification preferred.
• Bilingual English/Spanish preferred.

Responsibilities

• Develops and implements discharge planning within an interdisciplinary team.
• Identifies and removes barriers to post-acute care access.
• Collaborates with providers to optimize hospital days and length of stay.
• Provides clinical leadership to healthcare teams and ensures care plan goals are met.
