Job summary

Qualifications

• Graduate from an accredited school of nursing.
• Active Registered Nurse license in Connecticut.
• Case management certification preferred.
• Experience in acute care settings preferred.
• Bilingual English/Spanish preferred.

Responsibilities

• Develops and implements discharge planning within an interdisciplinary team.
• Addresses complex clinical care needs and removes barriers to post-acute care.
• Collaborates with providers to optimize use of acute care days and prevent delays.
• Provides clinical leadership to care teams and ensures goals align with the plan of care.
