Qualifications

• Graduate from an accredited school of nursing.
• Valid Registered Nurse License with the State of Connecticut.
• Previous experience in acute care healthcare setting.
• Preferred bilingual English/Spanish.
• Case Management Certification preferred.

Responsibilities

• Develops and implements discharge planning within an interdisciplinary healthcare team.
• Addresses complex clinical needs by identifying and removing barriers that prevent optimum post-acute care access.
• Collaborates with providers to identify discharge barriers and delays to optimize acute care hospital days.
• Provides clinical leadership to healthcare teams and ensures daily goals are met according to the patient’s plan of care.
• Supports seamless transitions of care for patients and families.
