Job summary

You will work to identify discharge barriers, optimize hospital days, and lead care teams while ensuring the patient’s plan of care is followed. Bilingual English/Spanish preferred.

Qualifications

• Graduate from an accredited school of nursing.
• Valid Registered Nurse License (Conn).
• Case Management Certification preferred.
• Bilingual English/Spanish preferred.

Responsibilities

• Develops and implements discharge planning within an interdisciplinary team.
• Identifies barriers to post-acute care access and removes them.
• Collaborates with providers to optimize acute care hospital days.
• Provides clinical leadership to care teams and ensures goals are met.
