Job summary

Hartford HealthCare's MidState Medical Center in New Britain, CT is seeking a licensed RN care coordinator for per diem weekend coverage. You will develop discharge plans with an interdisciplinary team and address barriers to timely post-acute care.

You will collaborate with providers to optimize hospital days and lead care coordination efforts, ensuring daily goals align with patient plans and outcomes.

Qualifications

• Graduate from an accredited school of nursing.
• Valid Registered Nurse License with the State of Connecticut.
• Case Management Certification preferred.
• Preferred bilingual English/Spanish.

Responsibilities

• Develops and implements discharge planning within an interdisciplinary healthcare team.
• Addresses complex clinical care needs by identifying and removing barriers to post-acute care.
• Provides clinical leadership to healthcare teams to meet patient plan of care goals.
