Hartford HealthCare Organization is seeking a licensed RN care coordinator to manage discharge planning and coordinate post-acute care within an interdisciplinary team at MidState Medical Center.

The role involves developing and implementing discharge plans, identifying barriers, and collaborating with providers to optimize hospital days and transitions of care. Per diem weekend coverage is required, with potential for growth and advancement within Hartford HealthCare’s network.

Qualifications

• Graduate from an accredited school of nursing.
• Previous experience in acute care healthcare setting.
• Valid Registered Nurse License with the State of Connecticut.
• Case Management Certification preferred.
• Preferred bilingual English/Spanish.

Responsibilities

• Develops and implements discharge planning within an interdisciplinary healthcare team.
• Addresses complex clinical care needs by identifying and removing barriers that prevent optimum access to needed post-acute care.
• Provides clinical leadership to their healthcare teams and ensures daily goals are being met according to the patient’s plan of care.
