Qualifications

• Graduate from an accredited school of nursing.
• Current R.N. license in the State of Connecticut.
• Previous experience in acute care healthcare setting.
• Case management certification preferred.
• Bilingual English/Spanish preferred.

Responsibilities

• Assess and treat patients along the continuum of care.
• Develop and implement discharge planning within an interdisciplinary team.
• Address complex clinical needs by identifying and removing barriers to post-acute care.
• Provide clinical leadership to healthcare teams to meet patient goals.
• Collaborate with providers to optimize hospital days and transitions.
