Job summary

Hartford HealthCare Organization is seeking a licensed RN care coordinator to assess patients along the continuum of care and to develop and implement discharge planning within an interdisciplinary healthcare team.

The role focuses on addressing complex clinical needs, identifying barriers to post-acute care, collaborating with providers to optimize acute care utilization, and providing clinical leadership to ensure daily goals align with the patient’s plan of care.

Qualifications

• Graduate from an accredited school of nursing.
• Valid Registered Nurse License with the State of Connecticut.
• Case Management Certification preferred.
• Preferred bilingual English/Spanish language skills.

Responsibilities

• Develops and implements discharge planning within an interdisciplinary team.
• Addresses complex clinical care needs by removing barriers to post-acute care.
• Collaborates with providers to identify discharge barriers and delays.
• Provides clinical leadership to care teams to meet the plan of care.
