Job summary

Hartford HealthCare MidState Medical Center is seeking an experienced RN care coordinator to lead discharge planning within an interdisciplinary team. You will identify and remove barriers to post-acute care, ensuring timely, efficient use of hospital days and strong collaboration with providers to meet each patient’s plan of care.

The role requires active nursing practice, coordination across care settings, and leadership to guide care transitions while maintaining high-quality patient outcomes.

Qualifications

• Graduate from an accredited school of nursing.
• Valid Registered Nurse License with the State of Connecticut.
• Case Management Certification preferred.
• Preferred bilingual English/Spanish.

Responsibilities

• Develops and implements discharge planning within an interdisciplinary healthcare team.
• Addresses complex clinical care needs by identifying and removing barriers that prevent optimum access to post-acute care.
• Works collaboratively with providers to identify discharge barriers and delays to optimize efficient use of acute care hospital days.
