Job summary

Wellstar Cobb Hospital is seeking an experienced Care Coordinator RN to assess transitional care needs, plan discharge, and coordinate care across the continuum. You will engage with patients and families to ensure needs are met and facilitate timely discharge in collaboration with the care team.

Qualifications

• Must have RN license and at least 1 year of nursing experience in acute care.
• BLS/CPR certification required.
• Strong communication and care coordination skills required.
• Nursing degree: Associates/Diploma required; BSN preferred.

Responsibilities

• Assess transitional care needs and plan discharge.
• Coordinate care across the continuum with the care team.
• Engage with patient and family to ensure needs, goals and discharge plans are met.
• Participate in interdisciplinary rounds and patient/family education.
• Collaborate with utilization review and physicians to facilitate timely discharge.
