Job summary

Wellstar North Fulton Hospital seeks an RN Care Coordinator for the Cobb Hospital Care Coordination team in Georgia. This full-time day shift role focuses on assessing transitional care needs and coordinating across the continuum to assure patient goals and discharge readiness.

The CC RN collaborates with physicians, care teams, and patients/families to promote efficient care, manage length of stay, and ensure appropriate resource utilization.

Qualifications

• Associate's Degree in Nursing from an accredited school.
• Georgia RN license required.
• Current BLS certification required.

Responsibilities

• Assess transitional care needs and coordinate across the continuum.
• Collaborate with PAS, financial counselor, and UM nurse to ensure insurance coverage.
• Meet with physicians and care teams to plan discharge and resources.
