Job summary

Wellstar North Fulton Hospital is hiring a RN Care Coordinator for a full-time day shift at Cobb Hospital. The CC RN assesses transitional care needs, coordinates across the care continuum, and engages with patients and families to assure goals are met.

The role emphasizes discharge planning, collaboration with physicians and the care team, and efficient use of resources. Requires GA RN license and 1 year acute care experience.

Qualifications

• Associate's Degree in Nursing is required.
• RN license (Georgia) required; single-state or compact license accepted.
• BLS certification required; instructor preferred.
• Minimum 1 year nursing experience in acute care.
• Strong written and verbal communication and teamwork skills.

Responsibilities

• Assess transitional care needs and coordinate across the continuum of care.
• Collaborate with physicians and care team to support discharge planning and patient flow.
• Document assessments, discharge plans, and care progression in medical records.
• Participate in interdisciplinary rounds and patient/family education.
