Job summary

Wellstar North Fulton Hospital is hiring a RN Care Coordinator for a full-time Day shift. The CC RN assesses transitional care needs, coordinates care across the continuum, and engages with patients and families to ensure needs are met.

The role plans effectively to meet discharge goals while managing length of stay and resource utilization. Responsibilities include psychosocial assessment, care progression, and collaboration with the care team to optimize discharge timing and care transitions.

Qualifications

• Associate's Degree in Nursing and Georgia RN license required.
• Minimum 1 year nursing experience in acute care.
• Excellent written and verbal communication skills.
• Ability to work under stress in a fast-paced environment.
• Knowledge of local, state and federal regulations.
• Ability to coordinate discharge planning with care team.

Responsibilities

• Assess patients' transitional care needs and coordinate care across the continuum.
• Engage with patient and family to ensure care needs are met.
• Lead discharge planning and progress through care progression.
• Collaborate with physicians, UR nurse, and care team in rounds.
• Document efficiently in the electronic medical record.
