Job summary

Wellstar North Fulton Hospital is seeking an RN Care Coordinator for the Cobb Hospital Care Coordination team. This full-time day position involves coordinating transitional care, assessing needs, and engaging with patients and families to ensure efficient discharge planning.

The RN coordinates across the care team, collaborates with physicians and case management, and ensures documentation is complete while managing resources effectively.

Qualifications

• Excellent written and verbal communication required.
• Self-directed with the ability to function well under stress.
• Strong assessment, interview, organizational and problem-solving skills.

Responsibilities

• Assess transitional care needs and coordinate across the continuum.
• Engage with patient and family to ensure care needs are met and discharge plans are ready.
• Collaborate with physicians, care team, and payers to facilitate timely disposition.
• Document in the medical record and track progress toward discharge goals.
• Participate in interdisciplinary rounds and post-acute referrals when needed.
