Job summary

Wellstar Health Systems, Inc. is hiring RN Care Coordinators with hospital experience to support the Cobb Hospital Care Coordination team. Day and night shifts are available, with relocation assistance for eligible candidates.

The RN Care Coordinator will assess transitional care needs, coordinate across the care continuum, and engage patients and families to ensure timely discharge planning and efficient care progression during acute admission.

Qualifications

• Associates Nursing required; Diploma or Bachelor's preferred.
• Minimum 3 years as staff nurse in acute care.
• Minimum 2 years as case manager in hospital or payer-based model.

Responsibilities

• Assessment based on screening; coordinate psychosocial needs and discharge plan.
• Care progression coordination with physicians and care team for timely care.
• Discharge planning management; develop individualized plans with physicians and family.
• Documentation of assessments and care progression in medical records.
• Precepting/mentoring of new hires and ongoing competency development.
• Professional development and contribution to department goals.
