Job summary

The Hospital of Central Connecticut in New Britain, CT is seeking an experienced RN Care Coordinator to lead discharge planning and coordinate care across the continuum. You will work within an interdisciplinary team to optimize patient flow and ensure timely, safe transitions.

Requirements include an active Connecticut RN license, BSN preferred, and strong communication skills. Bilingual English/Spanish is optional.

Qualifications

• Graduate from an accredited school of nursing, BSN preferred.
• Valid Registered Nurse License with the State of Connecticut.
• Case Management Certification preferred.

Responsibilities

• Develops and implements discharge planning within an interdisciplinary healthcare team.
• Addresses complex clinical care needs by removing barriers to post-acute care.
• Collaborates with providers to optimize length of stay and transition of care.
• Provides clinical leadership to healthcare teams and ensures daily goals align with the patient’s plan of care.
