Job summary

The Hospital of Central Connecticut is seeking an RN Care Coordinator to develop and implement discharge planning within an interdisciplinary team, addressing complex care needs and barriers to post-acute care. You will lead clinical activities, ensuring daily goals align with the patient’s plan of care and collaborate with providers to optimize hospital days.

Ideal candidates have an active Connecticut RN license and a BSN (preferred) with experience in an acute care setting.

Qualifications

• RN licensure in Connecticut is required or preferred.
• BSN degree is preferred for this role.
• Case management certification is preferred.

Responsibilities

• Develops and implements discharge planning within an interdisciplinary healthcare team.
• Identifies and removes barriers to post-acute care access to optimize patient flow.
• Leads clinical activities and ensures goals are met along the plan of care.
• Collaborates with providers to identify discharge barriers and prevent delays in hospital utilization.
