Job summary

MidState Medical Center in Connecticut seeks a Care Coordinator (RN) to coordinate and monitor the plan of care for optimal discharge planning and resource use. You will expedite interventions to secure prompt and safe discharge, acting as a change agent for care coordination.

The ideal candidate holds a CT RN license with 3–5 years of clinical experience; BSN is strongly preferred. Experience with utilization management and managed care is valued, and strong communication skills are essential.

Qualifications

• Current CT RN License required; 3–5 years of clinical experience preferred.
• BSN strongly preferred.
• Excellent written and verbal communication skills and patient assessment skills required.

Responsibilities

• Coordinate and monitor the plan of care with the interdisciplinary team to optimize discharge planning outcomes.
• Expedite value-added interventions to achieve prompt and safe discharge.
• Support CDMP documentation strategies and managed care processes.
