Job summary

The Hospital of Central Connecticut, part of Hartford HealthCare, is seeking an RN Care Coordinator to support discharge planning and seamless transitions of care. You will work with an interdisciplinary team to address barriers and optimize post-acute pathways, ensuring patient plans are followed and goals met.

Ideal candidates will have a Connecticut RN license, BSN preferred, and may hold Case Management Certification.

Qualifications

• Graduate from an accredited school of nursing; BSN preferred.
• Current Registered Nurse license in the State of Connecticut.
• Case Management Certification preferred.
• Preferred bilingual English/Spanish.

Responsibilities

• Develop discharge planning within an interdisciplinary healthcare team.
• Identify and remove barriers to post-acute care access.
• Collaborate with providers to optimize length of stay and post-discharge care.
• Provide clinical leadership to healthcare teams and ensure goals align with the plan of care.
