Job summary

The Hospital of Central Connecticut seeks an experienced RN care coordinator to assess and treat patients along the continuum of care. You will develop discharge planning within an interdisciplinary team and address complex clinical needs by removing barriers to post-acute care.

You will collaborate with providers to optimize hospital days and ensure seamless transitions. Strong communication and documentation skills are essential to support patient and family goals throughout care.

Qualifications

• Graduate from an accredited school of nursing; BSN preferred.
• Valid Registered Nurse License with the State of Connecticut.
• Case Management Certification preferred.
• Preferred bilingual English/Spanish.
