Job summary

The Hospital of Central Connecticut is seeking an RN Care Coordinator to join our acute-care team. The role focuses on discharge planning within an interdisciplinary healthcare setting and coordinating care across services.

You'll work with providers to identify barriers, streamline post-acute transitions, mentor staff, and ensure goals of care are met while maintaining a patient- and family-centered approach.

Qualifications

• Graduate from an accredited school of nursing; BSN preferred.
• Valid RN license in Connecticut.
• Case Management Certification preferred.
• Preferred bilingual English/Spanish.
• Experience in acute care or multiple healthcare settings.
• Knowledge of medical terminology and disease processes.

Responsibilities

• Develops and implements discharge planning within an interdisciplinary healthcare team.
• Addresses complex care needs by identifying and removing barriers to post-acute care.
• Collaborates with providers to identify discharge barriers and delays in order to optimize use of hospital days.
• Provides clinical leadership to healthcare teams and ensures daily goals are met according to the patient’s plan of care.
• Ensure seamless transition of care for patients and families.
