Qualifications

• Graduate from an accredited school of nursing; BSN preferred.
• Experience in acute care healthcare setting or multiple healthcare settings.
• Case Management Certification preferred.

• Addresses complex clinical needs by removing barriers to post-acute care.
• Collaborates with providers to identify discharge barriers and optimize hospital days.
