Job summary

The Hospital of Central Connecticut is seeking a Registered Nurse care coordinator to develop and implement discharge planning within an interdisciplinary team. The role focuses on addressing barriers to post-acute care and improving transitions of care for patients and families.

You will collaborate with providers to optimize hospital days, provide clinical leadership, and ensure daily goals align with each patient’s plan of care in a dynamic healthcare setting.

Qualifications

• Graduate from an accredited school of nursing; BSN preferred.
• Current Registered Nurse License in Connecticut.
• Case Management Certification preferred.
• Preferred bilingual English/Spanish.
• Knowledge of computer applications preferred.

Responsibilities

• Develops and implements discharge planning within an interdisciplinary healthcare team.
• Addresses complex clinical care needs by identifying and removing barriers to post-acute care.
• Collaborates with providers to identify discharge barriers and delays for efficient acute care use.
• Provides clinical leadership to healthcare teams to ensure daily goals are met.
• Ensures seamless transition of care for patients and families.
