Job summary

Qualifications

• Current CT RN License with 3-5 years clinical experience.
• BSN strongly preferred.
• Experience with utilization management and managed care.
• Excellent written and verbal communication skills and patient assessment skills required.

Responsibilities

• Coordinate and monitor the plan of care to achieve optimal resource utilization and discharge planning outcomes.
• Expedite value-added interventions to achieve desired outcomes and offer creditable alternatives for prompt and safe discharge.
• Serve as a change agent to improve care coordination, customer satisfaction, and financial outcomes.
