Job summary

The Hospital of Central Connecticut seeks an RN Care Coordinator to lead discharge planning and ensure smooth care transitions. You will work within an interdisciplinary team, addressing barriers to post-acute care and prioritizing timely, safe patient movement.

Required is a CT RN license; BSN preferred, with case management certification preferred. English/Spanish bilingual abilities are highly valued within a dynamic hospital setting.

Qualifications

• Graduate from an accredited school of nursing, BSN preferred.
• Valid Registered Nurse License with the State of Connecticut.
• Case Management Certification preferred.
• Preferred bilingual English/Spanish proficiency.

Responsibilities

• Develop discharge planning within an interdisciplinary team and coordinate care transitions.
• Address complex clinical needs by identifying and removing barriers to post-acute care access.
• Collaborate with providers to identify discharge barriers and optimize acute care bed utilization.
• Provide clinical leadership to care teams ensuring daily goals align with the patient plan of care.
