Job summary

Chapters Health System seeks an Admission RN to assess patients at the residence or in hospital settings and initiate care within our services. You will act as a liaison to Care Navigation and represent the organization in coordinating smooth transitions.

The role requires Florida RN licensure and BLS, with preferred hospice or ICU experience and EMR/EHR proficiency. Excellent communication and time-management skills are essential.

Qualifications

• RN license in the state where the employee will be working.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum of one year of RN experience; hospice or hospital experience preferred.
• Strong clinical assessment skills; ED or ICU background preferred.
• Experience with EMR/EHR systems.

Responsibilities

• Complete clinical admissions for all services.
• Explain services to patients and families, addressing questions empathetically.
• Educate patients/families on admission requirements and initial assessments.
• Initiate Plan of Care and physician orders.
• Coordinate care transitions with Care Navigation and facilities.
• Maintain documentation per policy and SBAR communications.
